Disappearing Items

Have anyone else's items been mysteriously dissappearing from their bag? For example, I was certian that I had a toxic orb in my bag, but, when I needed it, it disappeared. My Expert Band even disappeared while my Infernape was holding it! Is it just my game that this is happening to, or have other people had this problem too?
