How and where do you make custom avatars?

Find one on the internet.

If you want to make one yourself. Get an art program like PSP or Photoshop. Paint will do fine.
 
For actual CUSTOM ones, like made specially for you, you'd probably need to make your own, or find someone who does requests.

If you want to make your own, you'll need an image program, like Jonouchi said. For the ones I've done, I use a combination of Microsoft Paint, The GIMP, and Pokemon Project Studio. GIMP is a PhotoShop-type program, and it's free to download. If you just go to Google and type in "GIMP download" you should be able to find the site.

What I usually do is take the picture I want, crop it so that it's a square and the same dimensions all the way around, then do whatever editing, text adding, etc, that I want, and after I do that, just open it with GIMP and resize it to 100x100.

If you want one that's already made, LiveJournal's probably the best place to go. There's 2 or 3 different Pokemon icon communities there, along with communities for pretty much everything else in existance, so you'll probably find something you like there.
