How do you make chapter indexes?

You mean at the start of a thread, which has links to each chapter?
Well, as each following chapter (After Chapter 1) will most likely be in a different post, you just create a hyperlink to that post (You can find out the link to a specific post by clicking on the post number in the corner of a post). Then you just write one up, for example:
(Note: Replace "{"s with "[", I just wrote it like that so you can see the coding):
{u}Contents:{/u}
{b}Chapter 1:{/b} The First Chapter (See Below)
{b}Chapter 2:{/b} {url=http://link.com/example}The Second Chapter{/url}
{b}Chapter 3:{/b} {url=http://link.com/example2}The Third Chapter{/url}
