This has been happening a lot whenever I got mentions. I only logged in 2 minutes ago and I received a notification that I have "1 New Mention" and when I go to check it, it does not say who mentioned me or what thread it was from right away. I would usually have to wait until the next time I log on to see.
 
If someone deleted the post they mentioned you in, that could be why. Not sure why it would be happening other times though; not sure if others have been having issues with mentions.

It may help if you clear your cookies/cache.
