I'm sure this has been asked before -- but anyhow:

Why is it that all of the gym leaders look quite similar between the Anime and the games except for Blaine who looks TOTALLY different in the show than the games?
 
In the games, Blaine is hinted to be a (former) TR scientist who took part in the Mewtwo project... Since they really went outside of the TR plot and didn't really touch it outside of JJM's mishaps in the original series of the anime, they changed Blaine's backstory...
 
Blaine's appearance in the anime matches his original R/G artwork. He got the old man look in Japanese Blue, and that was carried over to the English games.

I think he has the same old man sprite in every game, though.
 
Pretty much all the Kanto Gym Leaders got redesigned.

I blame Takeshi and Kasumi, really. When the writers decided to have these two Gym Leaders follow Satoshi around on his journey, they probably thought it would be wildly inappropriate for him to be followed around by a shirtless guy and a girl in a two piece. So, the TV people gave them more clothes. And since they drastically reversioned the first two Gym Leaders, they probably figured they should just go ahead and reversion all of them.

So, Katsura becomes an old man with a pun-based fondness for wigs.
 
Koga, Erika, and Sabrina didn't get redesigned, and Surge is still more or less the same (he got a tan and apparently steroids, but overall...)

Giovanni though...I can't wrap my mind around that. Bright orange suit and for some reason his skull is shaped roughly like a walnut all of a sudden?

I'll give you Kyou, but the others just have their heads in common with their in-game equivalents, and that's exactly what Takeshi and Kasumi did too.
