Small Image-related Question

This seems like one of those questions that's either really good, or has such a blatantly obvious answer that I'm an idiot for even asking.

But my question is: How do you get the small images (like the Bulbasaur for this site) next to the URL in the address bar? I've been to countless HTML, CSS, and PHP sites...and either it uses none of these, or I've just consistently overlooked it. Hopefully someone can reveal where I went wrong.

Am I the only FireFox user that finds the Favicons on bookmarks tend to mess themselves up occasionally?

It's not that big a problem, considering that a Vbuilliten icon next to a completely Vbulliten free website wont hurt anyone but it is annoying in that you can't fix it without remaking the bookmark.

I havn't been able to suss the reason either, best I can come up with is that it just gets confused when loading sometimes.
 
Yeah, I've run across that too, but VERY rarely. As is, I only have two in my favorites that have, for lack of a better word, "glomped" onto an other site's favicon. And neither of the glompers had an original one to begin with. The rest seem to correct themselves if something happens.
 
Happens to me too, but I deal with it. ^^

Kinda like how GMail has a Pokeball for awhile o_O
