Mine is an obscure one. It's a kinda of zombie that is down the cellar in the movie Evil Dead. Her name's Henrietta. I wont post a pic as it's very scary. It certainly gave me nightmares throughout my childhood.

Medusa from Clash of the Titans (1981). The Medusa scene gave me nightmares when I was younger. Now I can admire how much work was put into the scene.
