Sometimes we mess up when playing games. There are small ones which can be easily corrected, and there are those that are facepalm inducing.

To start things off, I tried to find ways to get by in Zelda I, second quest. A couple times, I get into a situation in which I am pretty much trapped: no way to get out, no way to force a game over... If only I could quickly get out via leaving the game or quickly save without dying, which was possible back in the NES days. But nope, I am playing on Virtual Console, on my 3DS. To add to this, before I restored, I had half the Triforce Shards. After restoring, I got to the entrance of Dungeon 4, prior to obtaining Triforce Shard 4. Whoops!

Edit: And to top it off, I never knew about the changing controllers feature until I was told about it. :sweatlol:

I accidentally killed Gaius in my first playthrough of Fire Emblem: Awakening. I was so upset I didn't play the game for two months when I realised I couldn't recruit him. (Thankfully, in my second playthrough I learned how to recruit him.) Because Stahl was the one who killed him, I had a grudge against Stahl for a while (I kept Stahl unmarried on my first playthrough because of it. On my second one I was nice enough to pair him up with Cordelia).
 
One time I was at a friend's place playing Mortal Kombat Deadly Alliance and we were taking turns trying to clear the arcade mode. When I got the controller I was doing really well since I played MK a lot in the older titles but I got my ass handed to me when I fought Li Mei because I was too distracted by her jiggle physics. My friend knew what was going on and wouldn't stop giving me crap about it for days after. I was a hormone addled 13 year old give me a break.
 
Back in Brawl, I think I have detonated a Blast Box while being close to it twice. Both times I got KO'd, while one of them was match-ending, with my loss. I still facepalm thinking about those.

Also, broke a Smash Ball, while connecting with a spike hazard in Rumble Falls and getting KO'd, again in Brawl.
 
So I was playing Metal Gear Solid 2 on the Metal Gear Solid HD Collection. I'm in a locker room after having stuffed the body of a dead guard inside of one of them. Naturally, none of the other guards go into this room during their usual patrol, so I was free to sit a moment and monitor the radar until it was safe to sneak out. However, I was multi-tasking at the time, since I usually play games while having my laptop on at the same time. In a force of habit moment, my fingers had slipped to the buttons required to raise and fire my gun. (Don't ask how. I don't know either.) When I noticed where my fingers were at, it was too late. I had already released the shot button, causing Raiden to pull the trigger. The sound of the random gunshot immediately alerted the guards outside, forcing me to hide in one of the lockers. And this wasn't just any alert, it was a Room Clearing. Meaning every nook and cranny would be searched. (Usually meaning they'll look in the lockers if they can see you through the vents.) Thankfully, I had the foresight to try crouching inside of the locker I was hiding in, which kept me from being discovered thankfully. So despite my amazing screw-up, I got out just fine.
Shit though. Gotta wonder why that happened.
