I've only played the XBLA version but damsels (those women/men/dogs in red) give you a heart if you finish a level with them. You can also sacrifice enemies and damsels on an altar to gain useful rewards that eventually lead to massive gains in life. Seriously, I've had almost 20 hearts.

There are also lots of extra side things you can do.

Anyway, its free for the PC. There is an updated remake for the 360 that's actually well worth the price, adding hand drawn graphics and lots of new features.
